Administered by Davis & Ferber LLP, the Nassau County PBA has sponsored a comprehensive Legal Services Plan.

PBA SPONSORED DRIVERS SAFETY PROGRAM
PBA Sponsored Driver Safety Program
Members are entitled to attend Driver Safety Classes at the Nassau County PBA Office. Classes are available to all Full members, their spouses, and their dependent children. Completion of this course will allow you to receive a 10% discount on auto insurance.
REIMBURSEMENT PROCEDURE
We do offer an online reimbursement option. The fee for the course is $23.00 through the following link:

Please provide the following information when submitting for reimbursement:
- Certificate of Course Completion from Improv
- Serial number
- Contact phone number
- An actual or photocopy of a voided check. If submitting a photocopy, please ensure that both the routing number and account number are legible. The routing number is the pre-printed number along the bottom of the check. Handwritten account information will not be accepted.
The refund amount of $23.00 will be directly deposited into your bank account.

ABOUT THE PLAN
Full members (active members of the Nassau County PBA of the rank of Police Officer) should submit PAID receipts from the current calendar year to receive reimbursement.
REIMBURSEMENT CHECKLIST ** PLEASE FOLLOW CAREFULLY **
Please provide the following information when submitting receipts:
- Member name
- Serial number
- Contact phone number
- ALL receipts, from current calendar year only, totaling at least $300.00
- An actual or photocopy of a voided check. If submitting a photocopy, please ensure that both the routing number and account number are legible. The routing number is the pre-printed number along the bottom of the check. Handwritten account information will NOT be accepted.
For security reasons, canceled checks previously submitted were destroyed, so you MUST submit a new one each year.
ACCEPTED RECEIPTS
Annual Gym Memberships for member, spouse, and dependent children: Submit gym contract showing fees paid. New memberships and renewals are both accepted. If you pay monthly gym fees on a credit card, submit credit card statement(s)showing payment to your gym. Precinct Gym dues are also accepted.
Current-year Doctor/Dentist Co-Pays for member, spouse, and dependent children: Submit receipts for ANY doctor (MD, DDS, Chiropractor, EyeDoctor, etc.) showing co-pay made.
Current-year Prescription Co-Pays for member, spouse, and dependent children: Any receipt from your pharmacy showing the Rx*number with amount paid. *Over the counter medications are NOT applicable.
You can submit any combination of the above to reach the maximum $300.00 reimbursement. If a member submits receipts totaling less than $300.00, then he or she will be reimbursed for that amount.
Reimbursement will be deposited directly into your checking account.
PLEASE NOTE
The Nassau County PBA will issue only one reimbursement per year, per member, so receipts should be submitted all at once.
LASIK SURGERY BENEFIT
Full members (active members of the Nassau County PBA of the rank of Police Officer) are entitled to a partial reimbursement for Lasik Eye Surgery. Full members are entitled to $500 lifetime benefit per eye. Reimbursement is for Active members only.
1. Request a voucher from the PBA
2. Submit completed voucher with paid receipt
3. Once your claim has been approved, a check will be mailed to you
LAMAZE TRAINING
Reimbursement is for Full members only and their spouse.
-Lamaze training or refresher courses will be paid up to $ 100 per course, per child.
-Request a voucher from the PBA Office
-Fill out form completely and return it to PBA Office along with a paid receipt and certificate of completion.
-Once claim is approved a check will be mailed to you.
STOP SMOKING BENEFIT
Reimbursement is for Full members only and their spouse. Each will receive a $300 lifetime benefit to Stop Smoking.
-Request a voucher from the PBA Office
-Return completed voucher with paid receipts to the PBA office
-Once your claim has been approved, a check will be mailed to you

INNER IMAGING
Members are to obtain an Inner Image Voucher from the Nassau County PBA office 30 days prior to your appointment.
Body Scan (The Heart, The Lungs, The Abdomen and The Pelvic region) screens for disease and has identified more that 233 separate and distinct diseases. They have also had 17 forms of Cancer confirmed. The Cancers that have been confirmed using their Body Scan are:

BRACES REIMBURSEMENT
Full members (active members of the Nassau County PBA of the rank of Police Officer) are entitled to a one time reimbursement of $1,000 per member, per dependent for braces. This benefit is for Active members only.
- Request a voucher from the PBA
- Submit completed voucher with paid receipt
- Once your claim has been approved, a check will be mailed to you or you may provide a voided check for direct payment.
DENTAL BUY-UP REIMBURSEMENT
The PBA always seeks ways to help our members and their families. That said, we want to add to our members' welfare. For any member who chooses the Buy-Up program, the PBA will reimburse the member 50% of the plans cost each year.
- Please provide your NCPD pay stub showing your Name and Dental deduction
- Once your claim has been approved, a check will be mailed to you or you may provide a voided check for direct payment.
In addition, we will be offering $100yr for Full Members in the PPO/DHMO plan with proof of dental receipts. Please provide the following:
- Member Name
- Serial Number
- Contact phone number
