PBA SPONSORED DRIVER SAFETY PROGRAM: ASSOCIATE MEMBERS

PBA Sponsored Driver Safety Program

As part of the Members Benefit Fund, the PBA will offer Driver Safety Classes at the PBA Office at no charge. The class will be available to all Full Members, their spouses, and children. Completion of this course will allow you to receive a 10% discount on auto insurance.

Space is limited, so act fast once new dates are announced. To reserve your spot, call the PBA office at (516) 294-6230 or you can choose to take the course online (see below).

All Full and Associate Active members and their immediate families will be eligible for reimbursement by the PBA of the $23.00 discounted fee that is offered through the link below.

Retired and Half Members and their families are eligible to utilize the link for the discounted rate of $23.00 but will not be eligible for the reimbursement.

REIMBURSEMENT PROCEDURE

Please provide the following information when submitting for reimbursement:

• Certificate of Course Completion from Improv
• Serial number
• Contact phone number
• An actual or photocopy of a voided check. If submitting a photocopy, please ensure that both the routing number and account number are legible. The routing number is the pre-printed number along the bottom of the check. Handwritten account information will not be accepted.

The refund amount of $23.00 will be directly deposited into your bank account.

ABOUT THE PLAN

Associate members (active members of the PBA at the rank of Detective or Supervisor, not including active Partial Associate Members) should submit PAID receipts from the fiscal year (July 1st-June 30th) to receive reimbursement.

REIMBURSEMENT CHECKLIST
** PLEASE FOLLOW CAREFULLY **

Please provide the following information when submitting receipts.

• Member name
• Serial number
• Contact phone number
• ALL receipts, from fiscal yearly only, totaling at least $150
• An actual or photocopy of a voided check. If submitting a photocopy, please ensure that both the routing number and account number are legible. The routing number is the pre-printed number along the bottom of the check. Handwritten account information will not be accepted.

For security reasons, canceled checks that were previously submitted were destroyed. You MUST submit a new one each year.

ACCEPTED RECEIPTS

• Annual Gym Memberships for member, spouse, and dependent children.
  Submit gym contract showing fees paid. New memberships and renewals are both accepted. If you pay monthly gym fees on a credit card, submit credit card statement(s) showing payment to your gym. Precinct Gym dues are also accepted.
• Current Fiscal Year Doctor/Dentist Co-Pays for member, spouse, and dependent children.
  Submit receipts for ANY doctor (MD, DDS, Chiropractor, Eye Doctor, etc.) showing co-pay made.
• Current fiscal year Prescription Co-Pays for member, spouse, and dependent children.
  Any receipt from your pharmacy showing the Rx* number with amount paid.
  *Over the counter medications are NOT applicable.

PLEASE NOTE

The PBA will issue only one reimbursement per fiscal year, per member so receipts should be submitted all at once.

ALL PAPERWORK MUST BE SUBMITTED NO LATER THAN JUNE 30TH OF THE CURRENT YEAR.

PBA-SPONSORED PENSION ESTIMATE PROGRAM: ASSOCIATE MEMBERS

Active Associate Members of the PBA (excluding active Partial Members) will be reimbursed $100 towards a pension evaluation of their choosing. The benefit is a one-time benefit, same as Full Active Members. Members must provide a paid receipt as proof of payment to be reimbursed.

LASIK EYE SURGERY

Associate members (active members of the Nassau County PBA at the rank of Detective or Supervisor, not including active Partial Associate Members) are entitled to a partial reimbursement for Lasik Eye Surgery.

Associate members are entitled to a $250 lifetime benefit per eye. Reimbursement is for Active members only.

How to Request Reimbursement:

• Request a voucher from the PBA
• Submit completed voucher with paid receipt
• Once your claim has been approved, a check will be mailed to you

STOP SMOKING

Reimbursement is available for Associate members only and their spouse. Each is eligible for a $150 lifetime benefit to assist with Stop Smoking programs.

How to Request Reimbursement:

• Request a voucher from the PBA Office
• Return completed voucher with paid receipts to the PBA office
• Once your claim has been approved, a check will be mailed to you

Please Note: Chewing gum, patches, and any other self-help aids are not covered.

Members are to obtain an Inner Image Voucher from the Nassau County PBA office 30 days prior to your appointment.

Body Scan (The Heart, The Lungs, The Abdomen and The Pelvic region) screens for disease and has identified more that 233 separate and distinct diseases. They have also had 17 forms of Cancer confirmed. The Cancers that have been confirmed using their Body Scan are:
